     Boarding Agreement: 
    • Evergreen Animal Hospital (EAH) is not staffed 24-hours and my pet will be left unattended past the closing time of 5:30pm.
    • All pets must be up to date on core vaccinations (Dog: DHLPP, Rabies, Bordetella / Cat: Rabies, FVRCP) to be boarded at our facility. If my pet is due for any of these required vaccinations, they will be given them upon intake and I am responsible for the associated costs. 
    • All pets must be free of external parasites (fleas & ticks) when admitted for boarding. If any parasites are seen during the stay, my pet will be treated and I am responsible for the associated costs.
    • I understand that if my dog is ill or injured while boarded at Evergreen Animal Hospital, EAH will make every reasonable effort to reach me pursuant to the contact information I have provided. However, if EAH is unable to reach me, I consent to EAH administering prudent and appropriate veterinary care and I accept responsibility for any and all associated expenses.
    • All precautions will be taken to prevent injury, escape, or other life-threatening emergencies. I understand that the nature of animal husbandry is not guaranteed and EAH and staff will not be held liable for any problems that may arise during my pet’s stay.
    • We make every attempt to return all items that you leave with your pet; however some items may be lost due to changing bedding or cleaning the items. We cannot guarantee the return of your items.

    By signing below you agree and understand the policies listed and you authorize Evergreen Animal Hospital to care for your pet during their stay with us. You accept all financial responsibility for any and all charges incurred during your pet’s stay. Thank you for choosing our team to care for your pet during your absence.
